Healthy Youth Survey

Healthy Youth Survey

Sometime between October 6th and October 24th, 10th and 12th grade students at Eastlake will be given the Healthy Youth Survey during their English class.   The Healthy Youth Survey asks about how youth are doing, social relationships, mental health, substance use, risks for injury, and more. Communities, districts, schools, and public health agencies use survey results to inform policies and programs designed to support youth well-being. This survey is voluntary. For more information, please see the parent/student information letter here - HYS 2025 Parent and Student Information Letter.  Linked in the letter are sample questions for your review.

If you want to opt your 10th or 12th grade student out of the Healthy Youth Survey, please complete this Forms survey - https://forms.office.com/r/b0x9Lf3Ued

Thanks,

 Josh Trimmell
