How to register for a sport

1. Visit https://lakewashington-wa.finalforms.com/
2. Click LOGIN under the Parent icon.
3. Click INCOMPLETE FORMS.
4. Select the sport for which you would like to register. Your selection can be changed at any time prior to the registration deadline. Register for only one sport per season.
5. Complete each form and sign your full name in the Parent Signature field at the bottom of the page. After signing a form, click SUBMIT FORM and move on to the next form.
6. You will see  a “Forms Finished” message when all forms are complete.
7. An email will automatically be sent to the your students LWSD email account, prompting them to complete the forms requiring their signature. Parents, please do remind your student to sign their forms! If students can't access their LWSD email account they can log in directly to FinalForms and sign.

Students are required to complete a WIAA contract if they do not attend the school full-time. Please see the information in the section below.

Physicals are valid for two years from date of exam. Wondering when your students physical expires? Log into FinalForms and you will see the date listed below your students name.

The only paper form per LWSD is the Medical Eligibility Form (Physical).

Do not upload the 3 page "Pre-Participation Physical Evaluation" form to FinalFormms.

Please note: Doctors signature and clinic stamp are required on the form. Please review your forms prior to submission to make sure they are complete!

How to submit form - Completed forms are turned into Amy Hill in the Eastlake main office or can be uploaded directly into FinalForms as a pdf. To upload physical to FinalForms: Log into FinalForms and select the WIA PPE PHYSICAL form, directions to upload are on this form.

All students participating in a high school sport must purchase an $60 ASB card. These funds directly support the year-to-year cost of the athletic programs, such as: facilities, equipment, uniforms, game transportation costs, and more. If you attend a CHIOCE school and live in the Eastlake  boundary you will also be required to purchase an Eastlake ASB card. 

There is a LWSD $250 Participation fee for each sport. Fees are collected on a per sport basis with both individual and family caps. The family caps will apply for brothers/sisters who attend the same school. Fees must be paid at the time of turnout (after cuts are made for try-out sports). High school parents can pay student fees online or arrange with the Eastlake Bookkeeper Karen Guinasso to pay in person with cash. 

Families that qualify for Free and Reduced meals, may be eligible for a free ASB Card and to have sports participation fees waived. Apply here.

100% refund will be provided under the following conditions:

1. Student is cut from the squad by the coach
2. Student quits due to illness or injury prior to first contest
3. Student quits due to family moving prior to first contest

LWSD Eligibility Criteria & Grade check Dates - here

Grade checks are run throughout each sports season.  You must be passing 6/7 classes to participate fully in athletics.

• At the end of the PREVIOUS SEMESTER, fall student-athletes in grades 10-12,who do not meet the WIAA academic standard (failing more than one class) are ineligible through the fourth Saturday of September.
• A student may earn a “class-for-class” replacement earned in a regular, accredited summer school program or alternative educational program accepted by the school district.
• Per WIAA rules, incoming 9th grade students do not have to meet an academic standard to be eligible for the initial Fall grade check, other than having a previous semester of attendance. Incoming 9th grade students first grade check will be in early October.

If you attend any other school for ANY classes, you must provide the Athletic Office with your grades from that school by the day of the grade check.
