FinalForms

Athletics Registration is all done through FinalForms.

Please read this guide so you are familiar with the process.

1. Parents - Login into FinalForms and create your account or log into your existing account https://lakewashington-wa.finalforms.com/
2. Verify your students sports selections, then click on the blue button that says "Incomplete Forms". Please only select one sport per season.
3. To the right is a visual of what is required to be completed and signed by a parent. Anything RED is incomplete and must be clicked on, completed and electronically signed (at the bottom of each page). Once a form is complete it turns GREEN.
4. Any form that also requires your students electronic signature will turn ORANGE. For a student to sign they can either log into their LWSD email account, open the email from FinalForms and follow the directions to sign OR log indirectly into FinalForms using the address above. Students, make sure you read each form prior to signing it.
